We would like your first visit to our office to be a
pleasant experience. All the information we
collect will help us in filing your insurance claims and
help our doctors in evaluating your current condition so
that a treatment path can be created specifically for
you. For your first visit, we kindly ask that you provide
the following:
- Your current insurance card.
- A current picture I.D.
- Please bring any current diagnostic imaging from
your most recent MRI, CT, and/or X-ray. Imaging on
CD-Roms is preferred.
- All your images will be returned to you for your
records.
- Please bring any reports pertaining to these
scans. We will make a copy for your chart and
return the original documentation to you for your
records.
- You will be asked to complete a New Patient
Registration Form, which we will send to you in
advance of your appointment. If at all possible, we
request that this document be fully completed and
returned prior to your visit to allow our staff to
upload this information to your chart before the
doctor begins his examination.
